Metabolic Syndrome Canada
Metabolic Syndrome Canada (MetSC), a not-for-profit organization founded in 2011 with a mission to improve the long-term health of Canadians through effective diet and exercise lifestyle intervention. MetSC created the CHANGE Program for family medicine clinics to help people reverse metabolic syndrome and prevent progression to serious conditions such as diabetes, hypertension, and cardiovascular disease.
Pacific Northwest Division of Family Practice
Family Physicians in the Pacific Northwest have been leading the way with implementing the CHANGE Program in their communities as a cornerstone of their Primary Care Networks (PCNs).
University of British Columbia
The faculty of Kinesiology has added CHANGE Program training into their undergraduate and graduate curriculums including student work term placements in remote communities. The faculty of Land and Food systems has provided program placements for graduate Dietetics students.
