Making the CHANGE Program one of the cornerstones of our PCNs

The results of the initial implementation of the CHANGE Program in 2016 and 2017 has inspired the team to incorporate the CHANGE Program as a standard of care in their practices. The team is working together with communities across the Pacific Northwest to make the program available in all communities.

Our healthcare system has been supportive in helping us make the CHANGE Program one of the cornerstones as we develop our PCN model. Prescribing the CHANGE Program has changed the lives for so many of our patients and we are excited to make it available to even the most vulnerable people across the northwest.
